Tunisian prosecutors opposed freeing Libya suspect

TUNIS, Tunisia (AP) ? Tunisian prosecutors were opposed to the release of a suspect in the attack on the U.S. diplomatic mission in the Libyan city of Benghazi that killed the ambassador, an official said Thursday.

Ali Harzi, a 26-year-old Tunisian extradited from Turkey in October, was one of the only people actually detained over the attack, and at the time Tunisian authorities said they "strongly suspected" he was involved. A judge released him conditionally this week citing a lack of evidence, although he has to remain in the Tunis area to be available for further questioning.

Fawzi Jaballah, an adviser to Tunisia's Justice Ministry, confirmed Thursday the prosecutor's office opposed the release.

On Sept. 11, armed groups assaulted the lightly guarded Benghazi mission and killed U.S. Ambassador Christopher Stevens and three other Americans. Despite U.S. promises, there has been little news of progress so far in bringing the perpetrators to justice.

U.S. officials have complained about a lack of cooperation with the governments of the region, particularly Libya, in their ongoing investigation into the attack, saying most of the suspects remain free.

Mounting a coherent investigation is difficult for the Libyan government, especially in Benghazi, because authorities rely on the militias who fought and helped topple former leader Moammar Gadhafi and it is often difficult to draw clear lines between those providing security and those causing the instability.

Doctors develop new test for cancer screenings | KFOR.com ...

BALTIMORE, Md.?Doctors are putting a new spin on an old test to help spot deadly diseases where there are no screening tests.

The pap smear has proven to be a simple and effective test for cervical cancer and the drop in deaths from the disease has made it a top screening tool.

Now researchers are trying to apply the test?s success to ovarian and endometrial cancer.

Dr. Luis Diaz, associate professor of oncology at John Hopkins, said, ?One of the reasons we?re so excited about this research is that there are no screening tests for ovarian cancer and it?s such a deadly disease because it is detected so late.?

Researchers at John Hopkins University assembled a list of genetic mutations common in ovarian and endometrial cancer.

Then they developed a test that could spot these mutations in fluid taken from a pap smear.

Dr. Diaz said, ?In 100 percent of endometrial cancers, we can find the exact same mutation in a tumor and a pap smear sample.???

They were also able to detect nearly half of ovarian cancers.

One of the most significant findings is what they didn?t find; there were no false positive results.

So far, the test has only been studied in the lab.

Researchers hope to have a version for the doctor?s office ready in three to five years.

Studies show about 20,000 women are diagnosed with ovarian cancer every year and 40,000 are diagnosed with endometrial cancer.

STATEN ISLAND, N.Y. -- An 83-year-old man who died in his Charleston home last year is now being called a homicide victim after he took a fatal dose of a chemical found in antifreeze, the city medical examiner's office has ruled.

At first glance, Steven Mario Carnesi seemed to have died of natural causes on May 24. He had become ill, and was placed under hospice care at his 4223 Arthur Kill Road home with his wife and a nurse looking after him, according to multiple law enforcement sources.

And there may well have been no investigation into what killed him if not for suspicious family members at his wake, those sources said. Carnesi was being laid out at Colonial Funeral Home in New Dorp, and a family member "didn't like the way he looked," noted one NYPD source.

So they called for an autopsy, and a medical examiner took tissue samples for a toxicology test, those sources said.

On Dec. 10, the medical examiner officially ruled his death a homicide by "propylene glycol intoxication."

A spokeswoman for the medical examiner's office would not comment on this case specifically, though she said that cause of death investigations often include going to crime scenes and interviewing family members and others.

Even so, some questions remain as to what dose of the substance would prove fatal. Industrial grade propylene glycol is found in de-icing fluid and antifreeze, but other grades of the substance can be found in food, cosmetics and medications.

Propylene glycol is often sold as a "non-toxic" alternative to poisonous ethylene glycol, the main ingredient in antifreeze. Propylene glycol is mildly sweet-tasting, but is not considered harmful to humans -- except in large doses, when it can be fatal. It has been used in a number of high-profile murders.

The chemical is also a common ingredient in liquid detergents, paints and inks, but it could cause metabolic abnormalities and even death if ingested in "very high amounts," according to Sam Miller, assistant commissioner for public affairs with the city Department of Health and Mental Hygiene.

Toxicity has occurred with high doses of intravenous medications that use it as a solvent, Miller said in an e-mail to the Advance.

On May 21, three days before his death, Carnesi's wife, Pamela Carnesi, 54, posted the following to her public Facebook.com profile: "My husband of over 30 years is dying. I have hospice 16 hours day. So many people comin & goin, but I dont feel like its real, as if I'm not really here I feel so alone. God, I miss my mother!!"

And on Sept. 14, she posted: "I am now the proud owner of The Foxhole Mobile Home Park. Thank you, my wonderful husband, for everything. Love, your wife." She posted a picture of the business on Sept. 25.

Public records show Carnesi bought the property at 4223 Arthur Kill Rd. in 1996 for $60,000. His wife sold it to Clarendon Realty Holdings on Aug. 4 for $840,000.

The house sits near the edge of a 20,500 square foot parcel of property, adjacent to the Charleston Cemetery and a two-building commercial complex with offices, an auto-detailing shop and a recently opened knife-grinding business.

Attempts to reach Carnesi's wife in Florida were unsuccessful.

A man who answered her phone and identified himself as Tommy Donohue claimed to be the family's spokesperson -- he said Carnesi was under "around-the-clock hospice care," and noted that the entire family agreed to the autopsy at the request of his children, Steven and Kim Carnesi.

Carnesi's daughter declined to comment, and attempts to reach other family members were unsuccessful.

Crews narrow search area for skydiver in Wash.

NORTH BEND, Wash. (AP) ? A more focused search for a skydiver who vanished during a jump above Washington's Cascade foothills failed to locate the Florida man as darkness fell Saturday.

Crews narrowed the search area for 29-year-old Kurt Ruppert to the Mount Si area east of Seattle. King County sheriff's Sgt. Cindi West said they looked specifically in a quarter-mile-square area near the summit, but kept searching other areas as well.

Ruppert disappeared after he jumped from a helicopter at 6,500 feet on Thursday afternoon. The Lake City, Fla., man was wearing a special wing suit with fabric under the arms to allow him to glide like a flying squirrel.

West said they made the decision to narrow the search area based on the flight pattern of the helicopter he jumped from and information from his cellphone's location just before the jump.

Diabetes is a stubborn adversary

One by one, the diabetic patients reluctantly stepped on the scale in the basement of a South Los Angeles clinic. Nearby, a nurse scribbled numbers on a chart.

Camara January, 31, her round face framed by a sparkly headband, held her breath. The number stopped at 245 pounds. "That's not good," January said. Tracy Donald, 45, stepped up. Just under 240 pounds. "That is wrong," she said. Ramon Marquez, 62, tall and clean-shaven, methodically took off his watch, his cap and his shoes. 170 pounds. Ramon strips down to almost nothing, the others jokingly complain.

They are among those who come each month to the To Help Everyone (T.H.E.) Clinic, hoping to finally gain the upper hand on their diabetes, a disease wreaking havoc on their bodies ? and their community. Several have been hospitalized. Some have seen relatives die from complications. All have struggled to manage blood sugar levels that spike and plummet to ominous extremes.

A chronic illness that can lead to heart attacks, blindness and kidney failure, diabetes is exploding across the United States and raising enormous obstacles to the Obama administration's drive to control costs and reform the healthcare system. In California alone, the disease costs taxpayers and businesses roughly $24 billion annually. One in seven California residents has diabetes ? a 32% increase in the last decade.

"This is a train going in the wrong direction," said Dean Schillinger, a UC San Francisco Medical School professor and medical officer at the California Diabetes Program, run by the university and the state public health department.

Community health centers like T.H.E. Clinic are a crucial front line in the federal government's war against diabetes. They're charged with slowing the advance of the disease, which disproportionately affects poor patients whose care may be taxpayer-funded.

To do so, clinics must get millions of patients to change deeply ingrained eating habits, embrace regular exercise regimens and better manage their health to avoid unnecessary hospitalizations.

Many low-income, urban patients struggle with the willpower challenges afflicting diabetes sufferers everywhere. But their struggle is often complicated by limited understanding of the links between lifestyle and blood sugar levels, along with fewer safe, affordable and convenient options for healthy activities and fresh food.

"The approach of telling people they need to lose weight and exercise is clearly not working," said Martha Funnell, past chairwoman of the National Diabetes Education Program.

Like her mother and grandmother, 17-year-old Charnay Winbush has diabetes. She injects herself with insulin five times a day and wears a bracelet identifying her illness. But Charnay often tells people she isn't diabetic.

Without her injections, she feels weak and tired. When her blood sugar levels climb, she gets extremely thirsty and can't stop sweating. One incident sent her to the emergency room this summer. "I was scared," she said.

Doctors at T.H.E. Clinic tell Charnay she needs to lose weight, but she continues to gain. She keeps small bags of candy in the refrigerator next to her insulin. Counting calories brings unwelcome memories of math class, she says. The only exercise she likes is playing virtual tennis or bowling.

The diabetes battle permeates almost every aspect of T.H.E Clinic's routine. Patients are greeted in the waiting room by television programs about the disease. They are routinely tested for it, counseled about how to prevent it and referred to nutritionists and diabetic education groups to manage it.

Research shows such efforts can produce results. Patients surrounded by support and education programs do better at controlling their blood sugar, eat more healthfully and use their medication as recommended.

At the Saturday session, retired nurse Emily Moore handed out a soul food cookbook with healthy recipes. Patients shared about their favorite types of exercise. One woman said she liked dancing to disco music, another announced she was going to Zumba classes.

The sessions began as a diabetes education class but soon morphed into a less formal support group, said M.D. Donnell, education manager at T.H.E. Clinic. Patients say they come for the snacks, the nutrition advice and the camaraderie. James Hicks, whose leg was amputated after a diabetes-related infection, said he serves as a cautionary tale for others. "You never want to get this far," he said.

Despite the intense focus, a quarter of the clinic's diabetic patients haven't brought the disease under control, records show. Cynthia Francis, a physician assistant, dispenses sober warnings along with hope. "You have to be real with them," she said. "You have to tell them that managing their disease is a life-or-death situation."

Stephen Carter, a laid-off security director, is among those struggling to keep his diabetes in check. He cut out fried food, joined a gym and charts his blood sugar levels. But sticking to his diet and exercising regularly is "an incredible challenge," he said. "It's really, really hard to break habits you've had for 50-some odd years."
